Do you need help ensuring your loved one has a one on one socialization visit when you are not able to provide it yourself?

There may be many reasons why you are unable to visit your loved one:

  • You are going on vacation
  • You are experiencing your own health concern
  • You have work increase work demands
  • You have childcare demands
  • Your loved one is placed in a facility too distant from where you live

What will be provided during the visit:

  • provide social activity that your loved one enjoys
    • listening to music
    • Playing cards
    • being read there favorite book
    • watching their favorite show or movie with them
    • Reminiscing with a photo book provided by you
    • Engage in an activity identified by yourself that your loved one enjoys
  • Provide outdoor visits in warmer weather in nursing home’s designated area
  • Attend encouraged scheduled nursing home activity on day of visit to increase social interaction
