Have you completed your Advanced Care Planning Document?

It really doesn’t matter if you are 21 or 81. Everyone should consider having an advanced care directive completed. I of course think about this more due to my current age and having had Cancer. I did undergo a long surgical procedure which made me need to think about how I would want to proceed if something went wrong during the procedure.

My career choice is another reason that I am familiar with Advanced Care planning. It is extremely important during a emergent event to know what each of your patient’s plan is.

The fact sheet outlines the changes that have been added which you may not have on yours if you did it earlier in your life. You may want to review and answer what your wishes are for those specific new ones. It does make sense to review your plan yearly or when facing any change in your health as your decisions may change from when you first completed the form. Please note that a psychiatric advanced directive has been added.
