Ensuring skilled nursing facilities (SNFs) maintain a CMS rating of at least three stars in the CMS Five-Star Quality Rating System is crucial, especially with the upcoming changes in January 2026.

The introduction of the new model by CMS brings significant alterations, including the implementation of two payment policy waivers for participants in the new initiative known as TEAM.

Telehealth Services Waiver: Beneficiaries under TEAM can now access telehealth services without specific geographic or originating site constraints. Additionally, CMS waives the facility fee for telehealth services originating from the beneficiary’s home.

SNF 3-Day Rule Waiver: This eliminates the prerequisite for beneficiaries to have a minimum three-day inpatient hospital stay before transitioning to a SNF for Medicare coverage. Medicare will cover SNF stays for TEAM beneficiaries, even if discharged from the hospital in under three days. However, SNFs must maintain a CMS rating of at least three stars in the CMS Five-Star Quality Rating System for seven of the past 12 months to qualify.
